A preloaded spending card for qualified OTC items from network retailers. $50 loaded every quarter (3 months). Benefits do not roll over to the following quarter.

Before using the card for the first time, the member needs to activate it. Follow the instructions included on the sticker on the card. As soon as it’s activated, it is ready for use.
The OTC card can be used at participating retailers for any CMS-approved items.
The most up-to-date retailers and products are available in the My Total Benefits portal using the OTC card number.
The member can also place an order:
ONLINE: Log into My Total Benefits portal and go to online ordering.
PHONE: Call Member Outreach at 1-888-267-9046 (TTY 711) and have item numbers ready when calling.
MAIL: Complete the order form (available on pp. 40-41 of the Over-the-Counter (OTC) Catalog) and mail it to: AZ Blue Benefits Catalog Orders, 4613 N. University Drive, #586, Coral Springs, FL 33067.
For delivery, allow 5–7 business days from the time the order is placed.
Call within 60 days of placing an order for status/tracking questions, if the order has not been received, if a defective or damaged item was received, or for product questions.
Items purchased using the OTC card cannot be returned or refunded.
Balances left over at the end of the quarter (every 3 months) do not carry over.
