AZ Blue partners with Adobe Population Health to provide an in-home comprehensive health assessment at no cost to members.


How it works

  • Your loved one will meet 1-1 with a board-certified Nurse Practitioner (NP).

  • The visit lasts approximately 45 minutes

  • The NP will:

    • Complete comprehensive health assessment

    • Review medical history and medications

    • Recommend additional screenings and resources if needed

  • After the visit, the summary of visit will be mailed to both your loved one and their PCP


Other things to know

  • IHA does not take the place of PCP, it complements the care member receives from their PCP

  • AZ Blue Member Outreach can assist in scheduling at 1-888-267-9046, TTY 711


An In-Home Comprehensive Health Assessment (IHA) is an eligible health activity in the Blue Medicare Health Rewards program. Completing a IHA qualifies your loved one for a $50 reward. (or call the Member Outreach team at 1-888-267-9046, TTY 711), the reward will be loaded onto your loved one's AZ Blue OTC/Rewards Benefits Mastercard for use anywhere that accepts Mastercard.
