AZ Blue partners with Adobe Population Health to provide an in-home comprehensive health assessment at no cost to members.
How it works
The member will meet 1-1 with a board-certified Nurse Practitioner (NP).
The visit lasts approximately 45 minutes
The NP will:
Complete comprehensive health assessment
Review medical history and medications
Recommend additional screenings and resources if needed
After the visit, the summary of visit will be mailed to both the member and their PCP
Other things to know
IHA does not take the place of PCP, it complements the care member receives from their PCP

An In-Home Comprehensive Health Assessment (IHA) is an eligible health activity in the Blue Medicare Health Rewards program. Completing a IHA qualifies the member for a $50 reward. The reward will be loaded onto an AZ Blue OTC/Rewards Benefits Mastercard for use anywhere that accepts Mastercard.

IMPORTANT: The AZ Blue OTC/Rewards Benefits Mastercard is reloadable and will be used by the plan to load rewards, so make sure to hold onto it to use throughout the year.
