Routine vision exams and standard eyewear. They say that life begins at 40, but so does faulty eyesight! Poor vision impacts safety, mood, and thinking.

How To Receive

2 Routine eye exam per year
1 eye exam for glasses every two years
1 eyewear fitting every year
Benefit is limited to either one set of eye-glasses or contact lenses, but not both.
Eye glass includes a frame allowance of $300, plus standard lenses (i.e. single, bifocal, trifocal; does not include specialty lens (i.e. transition, tints, progressives, polycarbonate).
Contact lens is limited to $300 allowance.
Schedule an eye exam through the provider partner, Superior Vision, or through a network optometrist.

Attend the appointment and complete the eye exam.
Obtain a prescription for corrective lenses (eyeglasses or contact lenses).
Present the prescription to a network eyeglass or contact provider, if not being made in the optometrist’s office.
If needed, obtain a network optometrist referral to a network ophthalmologist for vision conditions requiring medical attention.
If transportation is needed to/from the appointment:
Call (877) 718-4219 (TTY: 711), M-F 8am-8pm, at least 48 hours in advance of the needed transportation.
Have the date, time, location, and reason for transportation ready when calling.
Receive authorization and guidance on scheduling the transportation service.
Get confirmation of scheduled transportation.
Use the transportation service to and from the appointment.
FOR THOSE WITH DIABETES: A retinal eye exam for diabetes is an eligible health activity in the VNS Health Rewards Program. Completing a retinal eye exam qualifies the member for a $15 gift card. Once the health plan receives the claim that this activity was completed, a gift card will be issued to the member. For questions regarding the Rewards Program or issuance of a gift card, contact the VNS Health EasyCare Team at (866) 783-1444 (TTY: 711).
Note that gift cards are generally mailed out about 3 months after the close of the quarter in which the activity was completed. For example, if an activity is completed in February, the quarter closes at the end of March, and the gift card will be issued in June. If an activity is completed in the fourth quarter of the year (October - December), the gift card will be issued in March of the following year.

Routine vision exams help to identify vision changes that may require prescription adjustments, of course, but they can also detect many other things—early sight loss, dry eye syndrome, astigmatism, corneal complications, cataracts, threats to the optic nerve, macular degeneration, glaucoma, and damage from diabetes. Many people believe that they see just fine, and perhaps they do. Most do not know, however, that they have actually lost their ability to see clearly at very close range or at farther distances. They may have also become so accustomed to issues with their eyes, that they have learned workarounds and accepted their difficulties as normal. Some eye complications, including serious threats to vision and eye health, are unknown until there is an irreversible problem and can only be caught early through a professional eye exam. Finally, modifiable factors that contribute to vision disturbances, such as certain medical conditions and medications, can also be addressed.
The plan covers several vision care needs.
1 routine eye exam every year
Includes glaucoma screening for those at high risk (family history, diabetes, African Americans aged 50 and older, Hispanic Americans aged 65 and older)
Includes diabetic retinopathy screening for those with diabetes
1 eye exam every two years for the purpose of getting corrective lenses
One pair of standard* eyeglasses (frames and lenses) OR contact lenses every year, but not both
Physician services (ophthalmology) for diagnosis and treatment of disease and injuries of the eye, including treatment for age-related macular degeneration
One pair of eyeglasses or contact lenses after each cataract surgery that includes insertion of an intraocular lens
If there are two separate cataract operations, the benefit cannot be reserved after the first surgery and two pairs of eyeglasses purchased after the second surgery
*Standard eyewear: eyeglasses include single-vision, bifocals, and trifocals; this excludes specialty lenses such as transition, tint, progressive, or polycarbonate; contact lenses include standard daily wear, extended daily wear, disposables, toric, or rigid gas permeable.
Coverage includes the replacement of lost or destroyed glasses and the repair or replacement of parts.
