Details on how to apply
Be sure to get an itemized invoice of services from the providers and facilities that provided care. The facility billing department can assist you.
Complete and submit an electronic claim at the VNS Health website, or complete and submit a hard copy claim to mail to VNS Health.
Use the itemized bill or invoice, along with the information above, to complete and submit a claim.

Members are covered for emergency care and urgently needed services outside the United States and its territories.
If emergency care or urgently needed care is received outside the United States, a form submitted for reimbursement is required. Supporting documentation for the reimbursement claim includes:

Date of Service
Provider name, type, address, and telephone number
Description of services
Amount paid
