Obtain an in-network provider order for home health services.
The provider will oversee home health care planning and services.
Contact the VNS Health MLTC Care Team at (888) 867-6555 (TTY 711) for prior authorization.
The provider ordering home health will most likely assist with prior authorization.
The VNS Health MLTC Care Team coordinates these services.
Select an in-network home health agency: Click on the Home Care tab on the page, then click on Certified Home Health Agencies (CHHAS).
Note that VNS Health also has a home health agency.

Home health care is a wide range of health care services, including medical care, provided in the comfort of home. It is prescribed by a provider who will oversee the care and is typically ordered following a significant illness or injury that impacted organ function, physical function, and/or mobility.
A Care Manager will coordinate all professional home health care services, which may include care from:
Nurses
Social Workers
Nutritionists
Physical Therapists
Occupational Therapists
Speech Therapists
These services are provided to help prevent, rehabilitate, guide, and/or support health in the home to improve mobility, functionality, condition management, and quality of life, and prevent hospitalizations or need for nursing home placement.
