Obtain a network provider referral for podiatry.
Locate a network podiatrist: Click on the Specialists tab on the page, then scroll and click on Podiatry.
Contact the VNS Health EasyCare Plus Team at (866) 783-1444 (TTY 711) for prior authorization.
The network podiatrist may help with authorization.
Schedule an appointment with the network podiatrist.
Attend the appointment.
Engage in recommended follow-up treatment and/or referrals.
If transportation is needed to and from the appointment:
Call (877) 718-4219 (TTY: 711), M-F 8am-8pm, at least 48 hours in advance of the needed transportation.
Have the date, time, location, and reason for transportation ready when calling.
Receive authorization and guidance on scheduling the transportation service.
Get confirmation of scheduled transportation.
Use the transportation service to and from the appointment.

Podiatry services are commonly needed for those who suffer from painful, debilitating, or deforming conditions of the feet, or for those who have chronic illnesses such as diabetes or peripheral vascular disease that affect the lower limbs and make the feet vulnerable to injury.
Conditions that a podiatrist may treat are (not limited to) diabetic foot care, foot injuries, toe deformities, toenail issues, skin infections, corns, calluses, bunions, bone spurs, ankle or heel pain, club foot, and plantar fasciitis.
Covered services include:
Diagnosis and the medical or surgical treatment of injuries and diseases of the feet (such as hammer toe or heel spurs)
Routine foot care for members with certain medical conditions affecting the lower limbs
6 routine visits per year
